Finding a trans-affirming therapist in Madison shouldn’t feel like rolling the dice. We provide affirming, identity-aligned care for transgender, nonbinary, and gender-expansive clients from our office in Glendale, WI, serving clients throughout the greater Madison area and all across Wisconsin via secure telehealth.

Madison has a reputation as one of the more progressive, welcoming places in the state, and in a lot of ways it earns that. There’s a real LGBTQIA+ community here, from campus life to long-established advocacy networks. But a city being affirming on paper doesn’t mean every therapist there actually gets it.
Plenty of trans and nonbinary people in Madison have sat across from a well-meaning provider who stumbled over pronouns, asked questions that felt more educational than therapeutic, or defaulted to a framework that treated gender identity as the problem to solve.
That gap between Madison’s open-minded image and the day-to-day reality of finding good care can be its own kind of frustrating. It leaves a mark, and it makes the next search feel heavier, making therapy feel like one more place where you have to manage someone else’s discomfort before you can focus on relieving your own.
You shouldn’t have to walk in bracing for microaggressions, ready to educate your therapist, or wondering whether you’ll need to defend your identity before the session even starts. That’s not care; that’s labor.

What trans-affirming therapy actually means
Trans clients are whole people, not a diagnosis
Being transgender or nonbinary is an important part of who you are, but it’s rarely the only reason someone comes to therapy. Trans and gender-diverse clients seek support for relationships, family stress, work, mental health, trauma, and life transitions — just like anyone else.
Trans-affirming care means understanding how gender identity shapes your experiences without reducing you to it. You don’t need to educate your therapist or make every session about being trans. We hold your gender as important context while focusing on what matters most to you.
Lived Experience and Deep Affirmation - Meet Your Clinicians

SJ (they/she) is a queer and trans nonbinary clinician who brings both professional training and lived experience into the therapy room. Their work offers clients care that feels grounded, validating, and genuinely affirming — and many find relief in working with a therapist who understands the terrain firsthand.

Caitlin (she/her) is cis, queer, and deeply committed to trans-affirming, anti-oppressive therapy. While she does not share the lived experience of being trans, her work is grounded in empathy, respect, and a commitment to creating therapy spaces where trans and nonbinary clients feel safe, affirmed, and celebrated.
Gender-related support (when relevant)
> Exploring identity and expression
We support clients ages 9 and up who are questioning, exploring, or clarifying their gender identity or expression — at any pace, and without pressure. We also support their families in understanding and supporting them.
> Navigating social, medical, or legal transitions
Some clients want support around coming out, coping with family’s reactions, transitioning at work or school, or navigating medical or legal changes. We’re here to ask you the right questions so that you can decide, for instance, whether a treatment or procedure makes sense for you. You are the expert of your own life and body.
Gender-affirming letters and support (if desired)
We provide WPATH SOC-8–aligned letters of support for gender-affirming medical care when requested. We write these letters for both ongoing clients and for folx who just need a letter (or second letter).
We only charge for the session(s), not the letter writing itself. We’re as angry as you are that you need a gatekeeping letter from a mental health professional in order to access medically necessary, lifesaving care. We hope that these gatekeeping policies in medical clinics will phase out in the coming years.
Sexual health and intimacy considerations
Sex therapy and gender-affirming care often overlap, particularly when dysphoria, body image concerns, or medical transition affect intimacy and sexual wellbeing. Our practice is led by Caitlin, an AASECT-Certified Sex Therapist, who provides ongoing training and consultation to our clinicians in trans-affirming sex therapy.
We frequently support clients with concerns related to body image and dysphoria, desire and arousal, and sexual functioning—including changes that may occur with hormone therapy. Our approach is affirming, non-pathologizing, and tailored to each client’s goals, relationships, and comfort level.
Fertility, family building, and life planning
We support trans folx who are parents — or who wish to become parents — in navigating fertility, infertility, and family-building decisions across different stages of life. This includes fertility preservation, pregnancy and parenting, donor or assisted reproduction, grief, and coping with medical or reproductive trauma.
Some fertility clinics require formal psychological consultations or assessments, particularly when donor gametes are involved; these requirements are based on clinic policy rather than state law. We do not currently provide these formal evaluations for fertility clinics (which are frankly gatekeeping for LGBTQ+ people, but that’s a tangent).
However, we do offer affirming support to help clients explore their options, understand emotional and relational dynamics, and make informed choices aligned with their values.

Intersectionality and lived context
There is no single trans experience.
Race, disability, religion, culture, socioeconomic status, body size, neurodivergence, and immigration status all shape how gender is lived and understood. We approach therapy with an intersectional lens and an awareness of how overlapping systems of power and marginalization can significantly affect mental health, safety, and access to care.
We practice from an anti-racist, anti-oppressive framework and remain attentive to how privilege and systemic inequities show up in our clients’ lives and in the therapy room. We take a stance of humility and ongoing learning, while recognizing each client as the expert on their own lived experience.

Not at all. We hold your identity as important context, not as the automatic center of every conversation. You set the agenda.
Is this a good fit for younger clients?
Yes. We work with gender-expansive clients in the Madison area starting at age 9, and we support parents and caregivers navigating this alongside them.
Are letters of support available even if I’m not a regular client?
Yes. We write aligned letters of support for gender-affirming medical care. We’re happy to talk with you about what that process looks like on our initial call.
Do you have an office?
Yes, we have an office in Milwaukee, WI. For our clients in Madison, WI, we offer secure, convenient telehealth options.
All prospective clients must schedule an initial $25, 15-minute phone call appointment with your preferred therapist. If you are unsure of which therapist you want to talk to please schedule with Caitlin. The $25 fee will be applied to your first session if it is determined to be a good fit and you choose to schedule.
